The DASP application is normally completed online — but a paper form still exists, and you can download a fillable PDF version here. DASPA's form is our version of the ATO's NAT 7204 paper application: it collects the same details, but instead of posting it to a super fund you return it to us, and a registered tax agent (TPB 26076969) prepares and lodges the claim for you. If you'd rather skip the paperwork entirely, the online form takes about 5 minutes.

How the paper route works
- Download the DASPA application form (PDF) above.
- Fill it in — type directly into the PDF on any device, or print it and write by hand. "I don't know my fund" is a valid answer; finding every account in your name is the core of the service.
- Sign it — by hand after printing, or type your full name in the signature name box (a typed name is accepted when you upload the form yourself).
- Send it back to us — upload a photo or scan of both pages at daspa.com.au/upload-form. We confirm receipt by email, then handle payment, identity verification and lodgement exactly as we do for online applications.
From there it's the same service as the online form: flat $149 + GST, every fund plus ATO-held super, refunded in full if we can't recover any super, and your money paid by the fund or ATO straight to your bank — typically within 28 days of a complete application.
Paper form vs online form
Honestly: the online form is better for almost everyone — it validates as you type, nothing gets lost in a photo of handwriting, and there's no printing or scanning. The paper form earns its keep when you're helping someone who isn't comfortable on screens, gathering details from someone without reliable internet, or you simply want a hard copy to work through before committing.

Can I use the ATO's paper form (NAT 7204) instead?
You can. NAT 7204 — "Application for a departing Australia superannuation payment" — is the ATO's own paper form, used to apply directly to a super fund when the online system can't be used. Going that route yourself means completing a form for each fund, meeting each fund's requirements (certified identity documents are commonly required, which from overseas usually means a notary), and chasing progress with each fund on Australian hours. It costs nothing but your time. What is ATO form NAT 7204?

If you completed the DASPA form, upload a photo or scan of every page at daspa.com.au/upload-form — do not post it to the ATO or your super fund. Our form appoints DASPA (Registered Tax Agent 26076969) to prepare and lodge the application for you, so it has to come back to us to do its job.
Do I need certified copies of my passport with the paper form?
Not with us. Whether you apply online or on paper, DASPA verifies your identity electronically with your passport and a selfie from your phone — no certified copies, no notary. Applying directly to a fund with the ATO's own paper form is different: funds commonly require certified identity documents, which from overseas usually means finding a notary.
Can I fill in the form before I leave Australia?
Yes — and it's smart to. Complete and upload the form any time; the claim itself can only be lodged and paid once you have left Australia and your visa has expired or been cancelled. Sending the form before you fly means we can lodge the moment you're eligible.
What if I make a mistake on the form?
Don't stress about it. A registered tax agent reviews every form before anything is lodged — if something is missing, inconsistent or unclear, we contact you by email or WhatsApp and sort it out. "I don't know" is an acceptable answer for super fund details; finding your funds is part of the service.
